Tailored Care Management

Tailored Care Management (TCM) helps adults and children with Mental Health (MH) and Substance Abuse (SA) needs connect with and coordinate the healthcare, behavioral health services, and community supports they need.

Each member is connected with a Care Manager who works directly with the individual and their family to understand their needs, preferences, and goals. We provide direct referrals to providers and specialists of your choice and follow up to make sure services are accessible, appointments and connections are completed, and barriers to receiving care are addressed.

Services include comprehensive care planning, coordination between medical and behavioral healthcare providers, referrals to specialists and community resources, assistance with benefits and other supports, help during hospital or other care transitions, and ongoing follow-up to make sure your services continue to meet your needs.

At Bridges of Hope, our goal is to make navigating care easier by helping you get connected to the right providers, services, and resources—and staying involved to make sure those connections actually happen.

SACOT

Substance Abuse Comprehensive Outpatient Treatment (SACOT) is a multi-faceted approach treatment service for adults who require structure and support to achieve and sustain recovery. This service operate at least 20 hours per week and offer a minimum of 4 hours of scheduled services per day with availability at least 5 days per week with no more than 2 consecutive days without services available.  The individual must be in attendance for a minimum of 4 hours a day in order for this service.

Peer Support Services-Community

Peer Support Services provide community-based support for adults with mental health and behavioral health needs. Services are delivered by trained Peer Support Specialists who use their own lived experience with recovery to provide encouragement, practical assistance, and support as individuals work toward greater stability and independence.

Peer Support Specialists meet individuals in their homes and communities and can assist with developing coping and recovery skills, accessing community resources, attending and navigating appointments, strengthening natural supports, building independent living skills, and overcoming barriers that may interfere with recovery.

Services are individualized and focused on the goals that matter to each person. Our Peer Support Specialists work alongside individuals—not just for them—to help build confidence, strengthen self-advocacy, and support meaningful progress toward recovery, wellness, and greater independence in the community.

Tele-health Outpatient Therapy

Bridges of Hope provides outpatient behavioral health therapy through secure telehealth services, allowing individuals to receive professional mental health and substance use support from the comfort and privacy of their home or another appropriate location.

Our licensed clinicians provide individualized therapy based on each person’s needs and goals. Services may address concerns such as anxiety, depression, trauma, stress, substance use, relationship or family challenges, behavioral concerns, and other emotional or mental health needs.

Telehealth therapy provides convenient access to ongoing treatment without the need to travel to an office. Our focus is on helping each person build coping skills, improve emotional well-being, work toward recovery goals, and create meaningful, lasting change.
